Refer a Mom

Who is Eligible?

​

From Mom with Love follows these eligibility criteria:​

​

• Terminally ill single moms who is the biological or adoptive mother of a child under age 18 who lives with them or under 21 and attending college on a part-time or full-time basis

• Single moms facing a terminal illness

• Terminally ill single moms not participating in a clinical trial

• Terminally ill single moms that have not received a wish or a monetary grant from any other organization

• Terminally ill single moms and child/children residing in the Greater Chicagoland area for the first year of the date of start of the organization.

Who Can Refer a Mom?

​

• Only terminally ill single moms can apply for the Gift. Family members, close friends, co-workers, peers, or medical professionals (doctors, nurse practitioners, social workers, hospice staff, etc.,) with detailed knowledge of the mom's situation may contact the mom and encourage her to apply.

Wish Application Guidelines

​

• Please read this section very carefully and follow all the instructions to complete the steps necessary to ensure your Gift is granted 

• You will find many answers to your questions in our Frequently Asked Questions section

• Please submit all required information; incomplete applications will be denied

 

We do not grant certain types of Gifts: 

​​

• Requests from individuals living outside the Greater Chicagoland area

• Requests from single moms with chronic illnesses – with the exception of individuals with a clinical prognosis of 12 months or less

• Any Gift request in violation of the rules, policies or procedures of our organization or that of our corporate partners, in effect from time to time

• Monetary limit for all Gifts

The Gift application must be printed, completed and submitted online via email to mom@frommomwithlove.org. Please submit all required information together: incomplete applications will be denied.
